Confirm where the claim stopped
Start with the last verified event. Was the encounter ready to bill? Was the claim submitted and accepted? Did the payer respond? Was a correction or records request actually completed?
Check the underlying evidence when a system label is unclear. A submission queue, completed task, or drafted correction does not necessarily prove the intended action happened.
When receipt cannot be confirmed, investigate the clearinghouse or payer status before sending another copy.
Separate waiting from inactivity
A claim may be legitimately awaiting payer processing after a documented action. Another may be waiting because nobody followed up on missing information.
Record what you are waiting for and the applicable response window. If that window has passed, define the next action. Avoid one universal number of days for every payer and claim.
“Pending” is useful only when it explains the reason, responsible person, and follow-up date.
Look beyond the oldest balances
Use aging to locate areas for review, then examine status and history. Recent claims can also be stuck before submission or acceptance.
Check for:
- Incomplete documentation or information requests.
- Rejections without a confirmed correction.
- Payer responses that have not been reviewed.
- Records or corrections prepared but not sent.
- Overdue follow-up after an action.
- Payments or responses not reflected in the working record.
This helps distinguish operational blockers from balances that are still within an expected processing window.
Assign the next step—not just a category
For each stalled item, record the blocker, last completed action, next step, owner, and due date. Escalate access or technical problems that prevent the responsible person from moving the work.
If several claims appear to share a cause, verify the pattern and assign a separate workflow fix. Do not assume every item in the same queue has the same problem.
A useful owner test is: Can someone other than the usual biller explain what this claim is waiting for and what happens next?
If the answer is no, visibility and ownership need improvement. Talk with a Brace expert about turning unresolved therapy billing work into an accountable follow-up process.
