What the Medicare 8-minute rule means
For Medicare outpatient therapy, timed services are reported in 15-minute units. The familiar thresholds are 8–22 minutes for one unit, 23–37 minutes for two, and 38–52 minutes for three. The total timed-code minutes for the day determine the billable unit count, subject to the applicable code and claim rules; this is not a license to round each timed service separately.
The calculation concerns timed services. Do not add untimed services to the timed-minute total. The record must support the services furnished, the minutes counted, and the units billed. Check the current CMS Medicare Claims Processing Manual, Chapter 5 and CMS Therapy Services guidance for the applicable date of service.
Do not apply Medicare’s method to every payer
The Medicare method is not a universal PT billing rule. Commercial plans may specify another timed-code methodology in a contract or policy. Confirm the payer’s instructions for the date of service rather than assuming that Medicare’s approach—or another payer’s approach—controls every claim.
Check the claim before submission
Confirm the payer and service date, identify timed versus untimed codes, reconcile the documented timed minutes with the total units, and verify required modifiers. If the service record and units do not reconcile, resolve the discrepancy before billing.
