PT vs. OT Evaluation Codes: What a Therapy Billing Team Should Check

Medicare’s therapy evaluation code families distinguish physical therapy from occupational therapy: commonly, 97161–97163 are PT evaluations and 97165–97167 are OT evaluations. Confirm the code, discipline, documentation, and payer-specific billing rules for the service; a code family alone does not establish coverage or payment.

Therapy billing reference material organized by discipline and code family

Start with the discipline and the record

For Medicare therapy billing, the commonly used evaluation code families are distinct: physical therapy evaluations use 97161, 97162, or 97163; occupational therapy evaluations use 97165, 97166, or 97167. The levels reflect the evaluation’s documented complexity under the applicable coding rules. Verify current code descriptions and payer guidance before billing; this short overview is not a substitute for the authoritative code set.

Do not select a code from the discipline label alone. The record must support the service and the code selected. Confirm that the rendering discipline, plan of care, documentation, and claim details align with the applicable requirements.

A useful billing-team check

Before submission, confirm:

  • The correct discipline and evaluation code family were selected.
  • The documentation supports the level billed and the service performed.
  • The clinician’s credentials and plan requirements are represented accurately.
  • Any payer-specific edits, modifiers, or claim instructions have been checked against current guidance.
  • The claim is routed to the correct payer and any needed enrollment or billing setup is in place.

If a claim rejects, identify whether the issue is code selection, missing documentation, provider or location setup, or a payer-specific edit. Correct the actual cause rather than changing a supported code merely to get a claim through.

Be careful when extending this to other disciplines

Speech-language pathology and other services have their own coding and documentation considerations. Do not assume PT or OT rules transfer directly. Check the current code set, Medicare policy, and the relevant commercial payer’s instructions for the discipline and service.
